First Visit
Your initial appointment usually will consist of an examination and review of your treatment options. Usually, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A complete and thorough medical history questionnaire
- A list of medications you are presently taking.
- If you have dental insurance, please be sure to provide the office with the most up-to-date identification numbers, etc. in order for us to obtain a verification of benefits at least one week prior to your visit.
- Please bring all the necessary completed forms as well as your valid government-issued identification (i.e. Driver’s License, State ID Card, Military ID, Passport, etc.) . This will save time and allow us to help you process any claims efficiently.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, congenital heart conditions, artificial heart valves, artificial joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, antiplatelet, chemotherapeutic bisphosphonates, radiation treatment, etc.). Please also be sure to notify our office and list any allergies to materials, medications, food, etc.
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.

Our office uses state-of-the-art intraoral digital 3-D scanning for treatment planning. It’s quick, easy, and painless! This scan eliminates the need for impressions and gives superior accuracy when it comes to recording dental conditions and factors such as a patient’s bite and overall unique dental anatomy of each individual patient. We use this tool to help fabricate dental prostheses, clear aligners, occlusal guards, and so much more!
